The following information must be provided along with this form as it applies to you:
- Verification of income for all applicants
- Referral letter from a previous Landlord (if currently renting)
- Medical Assessment Form completed by your Doctor
- Notice of Assessment from Revenue Canada from current income tax filed
Your application must be signed in the presence of a Commissioner for Oaths in and for the Province of Alberta. This service is provided at our office, free of charge, by appointment. Please call (403) 627-3833 ext. 3

Complete ALL questions supplying ALL of the requested information. If a question does not apply to your situation, mark N/A in the section. Space is provided for any other information you would like us to be aware of.
You must bring with you, the following information along with this form:
- Verification of income for all household members. This can be a copy of your current paystub or a signed letter from your employer for EACH working member in your family stating the rate of pay, number of hours worked per week, total earnings and commencement date of employment.
- Copy of most recent income tax return filed or Notice of Assessment
- Referral letter from a previous Landlord
- Social Assistance or AISH – a copy of Eligibility
- WCB or EIC – a statement confirming your weekly benefit and for how long
- Form of photo ID (Driver’s License, Passport)
- If you are a student, a letter from the registrar of your school verifying your registration as a full-time or part-time student. This is required for household head, spouse and all dependents over the age of eighteen (18) years.
Applications will not be accepted unless they are complete and all questions answered.

Rent Assistance Benefit Program

The Government of Alberta has redesigned its Rent Supplement Program to meet the needs of more Albertans, including working families and those most in need.
Starting April 1 2021, we are accepting applications for the Rent Assistance Benefit. Formerly, the Direct to Tenant Rent Supplement, this long-term benefit will continue to subsidize low income Albertans.
